What is a varicocele?ما هي دوالي الخصية؟

A varicocele is an enlargement of the small veins inside the scrotum — the pampiniform plexus — that drain blood from the testicle. In much the same way that varicose veins can develop in the legs, the valves within these veins may not work efficiently, allowing blood to pool and the veins to swell. Most varicoceles are harmless and do not require treatment. However, in some men a varicocele can be linked to a dull scrotal ache, a feeling of heaviness, or visible swelling, and in some cases it is one of the factors considered when investigating male fertility. Where a varicocele does cause symptoms, the most common is a mild, aching or dragging discomfort in the scrotum that tends to be worse after standing for long periods or towards the end of the day, and eases when lying down. Some men notice a soft swelling that has been likened to a "bag of worms," or a difference in size between the two sides. A varicocele may matter more when it is causing persistent discomfort, when it is associated with a concern about fertility, or — in adolescents — when it appears to be affecting the growth of a testicle. A varicocele is one of the more frequently identified findings when male fertility is investigated, and in some men it can be associated with changes in sperm quality, possibly related to a small rise in temperature around the testicle. It is important to keep this in perspective: many men with a varicocele father children without any difficulty, and the presence of a varicocele does not in itself mean there is a fertility problem. When fertility is a concern, a thorough assessment — including a semen analysis — helps to clarify the picture and whether treatment may be worthwhile. When a varicocele is treated, it is usually carried out as a day case, and most men go home the same day. Recovery is generally straightforward: a short period of rest, supportive underwear, and avoiding heavy lifting and strenuous exercise for a brief time all help the area settle.
